Transition Age Youth Task Force

Transition Age Youth – young people, age 16-28,  who have had difficulties in life or who are transitioning out of a system of care – often don’t get the assistance they need to become healthy and independent adults.  These may be youth in foster care, those with mental health or substance use problems, those who are involved in the juvenile justice system, homeless youth, youth who have been incarcerated or have mentally ill parents, those in special education, or those who have dropped out of high school.


Programs and Services for Transition Age Youth

MHA-Hawai`i convened a task force to find out what resources exist for these young people, and identify gaps in services.  We developed a listing of all programs that can be of help to transition age youth, called a “Resource Map,” which you can view at www.hawaiitay.blogspot.com. When you're at that site,  go to "TAY Resource Map as of 9/2008," in which you'll  see service area headings across the top; once you select a service area, scroll down and you'll see that the services are organized by island.  Currently several organizations are working to complete this so it can become a hands-on useful Resource Guide for Transition Age Youth and people who work with them.
